At Link Allied Health we respect every individual’s right to privacy and we are committed to maintaining the privacy and confidentiality of your personal information in accordance with the National Privacy Principles in the Privacy Act 1988 (Commonwealth).

Storage and Access
When you make a referral we collect personal information, such as names, e-mail addresses and phone numbers and details about your NDIS plan (e.g., your NDIS plan number, and plan dates).
When you participate in services with us (e.g., assessment and therapy) we collect confidential and sensitive personal information about you (e.g., via Zoom meetings, face to face contact, and phone calls usually when we are in the process of setting up or providing services). This includes information about your disability and health and well being, as well as any reports you you wish to share with us.
Your information is stored electronically in secure databases, and we take all reasonable steps to protect personal information from misuse, interference and loss, unauthorised access, modification or disclosure.
We will not use your personal information for any other purpose than to complete a referral and provide intervention within our service, and will only disclose it to others (e.g., to refer you to another provider as part of our service) with your consent.
Authorised workers at Link Allied Health are provided with access to client's personal information. Where personal information is no longer required (e.g., a referral does not proceed) or where required by law, we will securely destroy or de-identify information in accordance with legal requirements for retention and disposal. For clients who participate in our service (e.g., complete an assessment and receive a billed service) we are required to retain this information in our electronic record for seven years.
Accessing your information
Individuals may request access to the personal information we hold about them, in accordance with the provisions of the Privacy Act, Link Allied Health will provide supervised access to an individual’s personal information.
